Dale is survived by parents Kelly and Laura Cook; brother Nathan (Trina) Cook; nieces: Dharma, Breya, Tenly and Riley; grandparents Bill and Betty Banwell; aunts and uncles: Kevin Banwell, Tanya (James) Morash, Dianne (Rick) Gosselin, Karen (Ray) Bulbeck, Sherree (Dave) Clark and families; as well as numerous other relatives and many dear friends.
